Telehealth Appointments

Connect Within 15 Minutes of Your Scheduled Appointment

This telehealth portal is for patients and their authorized representatives who have an appointment with one of our providers today.

If you need to reach us for any other reason, please contact our Patient Care Team directly.

Step 1. Consent Forms

Please complete your consent forms before your appointment.
Incomplete forms may result in your appointment being canceled.

Who needs to complete consent forms?

  • All new patients

  • Patients having their first appointment with us (even if you’ve been seen in the hospital)

  • Patients who last submitted consent forms over one year ago

How to complete your forms:
Log in to your Piedmont MyChart account:

  1. Navigate to "Visits"

  2. Select your upcoming appointment

  3. Complete the eCheck-In process

  4. At the end, you’ll be prompted to sign the following three consent forms:

    • HIPAA Consent Form

    • Treatment & Telehealth Consent Form

    • Financial Release Consent Form

Step 2. Getting Started with Your Telehealth Visit

Please select the provider you are scheduled to see today.
You can confirm your provider's name by checking your Piedmont MyChart account.

Once selected, a new page will open where you’ll enter the patient’s name.

Important: When prompted, be sure to click "Allow" to enable access to your device’s camera and microphone.
